Just a quick update on my case. I've been waiting for the DQ to arrive as there was an entry on the HM Courts and Tribunals Service that it was sent on 27/04/26 however, to date I haven't received anything either electronically or in the post and should have by now. Today I checked the portal again and saw the claimant apparently filed their DQ on 11/05/26, although again I have not received a copy.
I initially tried contacting the HM Courts Tribunal Service via the number on the telephone claims form to notify them I had not received anything but after a lengthy wait on the telephone it said I was 34th in the queue so I decided not to wait and just to upload the form via your link and completed it following your advice.
I have sent the N180 to HM Tribunals Service and copied in DCB Legal (and myself). I believe I just wait now for them to confirm receipt and to be contacted regarding a mediation appointment as mentioned in your guidance. I assume I'm not missing anything major if I haven't had a copy of DCB Legal's DQ, although appreciate they should have sent me a copy.
